You read the first two lines of the case and your brain already knows the answer.

Clinical reasoning often begins with pattern recognition. You hear a few key symptoms and your mind quickly matches them to a condition you have studied before. That early hypothesis is not the problem. It is a normal cognitive shortcut.

Instead of generating multiple differentials, you start collecting evidence only for the first diagnosis. Lab results are interpreted through that lens. That initial label feels reasonable.

Cardiac enzymes were not considered initially because the first explanation felt sufficient. The possibility of an atypical presentation of acute coronary syndrome was not actively explored.

The goal is not to eliminate your first impression. It is to prevent it from becoming final too early.

Force Yourself to List Three Differentials
Even if the first diagnosis feels obvious, pause and write down at least three plausible alternatives.

Clinical reasoning is not a race. In training, depth matters more than speed.
Use Structured Case Frameworks
Unstructured thinking increases cognitive bias. A consistent framework reduces it.
